Our physicians request that you be referred to our office by your current primary care physician, provider or specialist.  This will enable our physician to speak with your provider concerning your condition and if necessary to order imaging studies and tests prior to your appointment.

For patients who are self referred we will need a copy of your current medical records for our doctor to see before we can set up an appointment.  This will help us give you an appointment in a timely manner.
